I enjoyed class and even asked for extra days. I feel like all of the workers truly care about you with no judgment. I'm so grateful there's an alumni meeting so that I can check in with everybody.

I went to IOP programs before, but Journey Pure was by far the most helpful. The staff and therapist actually cared, and I am in contact still to this day. I transitioned back into daily life sober and I'm still sober today.

I left inpatient treatment, came home to Louisville and started IOP. It was a very nice transition back to home. They helped me in a time where I needed support. My therapist was amazing and was very sad when our time was up. I wholeheartedly recommend.

Everyone was compassionate, professional and understanding of my addiction and its effect on my life and family. I was treated with dignity and respect. They helped me through detox, supported me throughout IOP and continue to follow up with me today.

Louisville IOP Program Questions

The real question is...are you ready to try something different? Here are some answers to help you feel more comfortable saying yes to reaching out.
1

Can't I just get sober on my own?

If you’re reading this page, you’ve already tried to get sober on your own. Drugs and alcohol hijack our brains in a way that makes it impossible to “just stop.”

  • Get a variety of coping skills – There’s no one thing that someone can say or teach to keep everyone healthy long-term. Instead, get exposed to all evidence-based methods, so you’re able to stay healthy as life happens and changes.
  • Be held accountable – Good intentions or even days of sobriety isn’t the same as creating a lifestyle and support network where sobriety comes naturally. We’re too used to hiding or lying to the people in our life, and that only makes us feel worse.
  • Underlying issues hold you back – Issues like depression, anxiety, stress, relationship conflict and past trauma will keep you locked in the cycle until properly diagnosed and treated too.

It’s OK to get help. And, you can do it right here in Louisville.

2

What is the Louisville clinic schedule?

IOP Program
The average person spends 4-6 weeks with the Louisville IOP team, with options to continue one appointment a week (pending availability).

  • Group therapy – usually 3 times per week, held Monday through Thursday with morning, evening and virtual options
  • Medical doctor appointments – as needed for physical issues and medications
  • One-on-one counseling – usually weekly at your convenience
  • Continued accountability coaching – regular calls from a Certified Peer Recovery Specialist (CPRS) during and for one year after you graduate from the Louisville program

Medication-Assisted Treatment (MAT) Program
You can participate in MAT (Suboxone / Buprenorphine Maintenance) in conjunction or as a stand-alone program. This program involves medical appointments that decrease in frequency over time.

  • Month 1: Weekly
  • Month 2: Bi-weekly
  • Month 3+: Monthly

Usually, we work with you on a plan to taper off Suboxone within one year, but longer-term patients are allowed on a case-by-case basis.

3

Is outpatient treatment the right fit for me?

Honestly, most people in addiction need inpatient rehab. The success rates are higher.

But…outpatient is ideal if:

  • The problem was just noticed.
  • There was a recent relapse after months or years of sobriety.
  • You’re transitioning from inpatient rehab back to the “real world.”

If you don’t meet the criteria:

  • Unless outpatient would be dangerous because of the extent of the addiction, people that don’t meet the criteria are still allowed to enroll in outpatient.
  • It’s much better than doing nothing.
  • Even though you might not “want” to, consider rehab.

What about my family and work obligations?

Your family and your work want you sober – able to be present and happy.

IOP and Suboxone Maintenance programs work around your schedule, so you can keep your job and your home life.

You already know that your stability is the priority right now. It’s worth figuring out how you can show up to your appointments, and keep showing up.

5

How bad is the addiction problem in Louisville?

Drug overdose deaths increased 89% in Louisville in 2020 versus 2019, driven by the opioid Fentanyl. Fentanyl is found in more drug tests in Kentucky than in any other state. And, Jefferson County has the highest opioid abuse rates in the state.

The binge-drinking rate and the percent of drinking deaths that involve alcohol is higher for Jefferson county than the rest of the Kentucky.

So…the addiction problem is in Jefferson County, but you can get help right here in Louisville.

Mom of Krista P.

When I made the first phone call, I had so many questions. I had no idea what to expect. I'd never been through anything like this before. They answered all my questions and even called me back to make sure that I didn't have any more questions.

They were willing to talk to me the entire way, the two hours I was driving to get there. They said, "We'll stay on the phone with you if we need to." I just felt special, honestly, at a time where I didn't feel special about myself and I really needed that in that moment.

For everyone

Naltrexone (Vivitrol, Revia, Depade) is non-addictive. The medication blocks the positive effects of alcohol and drugs to discourage relapse, while also reducing cravings.

It can be started day 7-10 of sobriety and has no serious side effects. Because of cost, it's usually used for 1-12 months, but could be taken forever.

In shot form (Vivitrol), it’s administered once a month at the clinic, eliminating the option to skip or mistake the dose. If not covered by insurance Vivitrol is $1,000 per monthly shot, though they offer a savings card. The other options are Revia and Depade. These daily oral pills are around $100 a month.

Gabapentin (Neurontin, Gralise, Horizant) is a medication that has been studied recently for cravings. While research is ongoing, the takeaway is that Gabapentin is addictive and rarely the right choice.

For alcohol

While Naltrexone is usually the first choice, the following medications are also FDA-approved specifically for alcoholism:

  • Acamprosate (Campral) reduces symptoms of long-lasting withdrawal and cravings, such as insomnia, anxiety, restlessness and generally feeling unwell. It must be taken three times a day.
  • Disulfiram (Antabuse) interferes with the body’s ability to absorb alcohol. Instead of feeling “good”, alcohol will cause unpleasant reactions such as nausea and possibly be dangerous. Those who have shown the ability to stay sober for an extended period with recent relapse are ideal candidates.
  • Topiramate (Topamax, Qudexy) is an FDA-approved medication for seizures used “off label” to also reduce alcohol cravings due to initial promising research.

For opiates

In addition to Naltrexone, these medications are FDA-approved to treat heroin or pain pill addiction:

  • Buprenorphine + Naloxone (Suboxone) helps prevent withdrawal symptoms by replacing the effects of opiates with a lesser effect that does not produce a “high.” This is typically administered for 7-10 days or a longer maintenance program. This medication is addictive but poses a much lower risk.
  • Methadone (Dolophine, Methadose) is a more dangerous version of the above medication that is no longer recommended.

For underlying issues

All other physical and mental health issues (like depression and anxiety) need to be treated – whether they helped fuel the addiction in the first place or came as a result of the addiction.

Common mental health prescriptions include:

  • Fluoxetine (Prozac, Sarafem) or Bupropion (Wellbutrin) for depression
  • Aripiprazole (Abilify) for mood and bipolar
  • Trazodone (Oleptro) or Hydroxyzine (Vistaril, Atarax) for sleep and anxiety
  • Paroxetine (Paxil, Pexeva, Brisdelle) or Sertraline (Zoloft) for major depress, PTSD, OCD
  • Oxcarbazepine (Trileptal) for mood stabilization
